Only 7 of the 33 are known to have gotten everything back. In 6, the backups were hit too, or there weren't any. Before an agent runs migrations, check that its credentials can't delete your backups, and that you have some.

Attackers using an AI-assisted offensive security framework got into a developer-platform company's sales and support tools
Agent-assisted attack
WorkOS says its investigation 'identified the use of an AI-assisted offensive security framework to automate reconnaissance and exploitation attempts', and that an attacker got unauthorized access to an internal tool used by its sales and customer-facing teams (found Oct 30). CRM contacts, account information, call and Slack excerpts, and product-usage metadata were affected; no evidence production services or application data were accessed.

Attackers using agentic techniques accessed a blockchain analytics firm's internal systems and customer user data
Agent-assisted attack
Chainalysis says an external actor used 'offensive agentic techniques' to gain unauthorized access to its systems and internal tools in October 2026, exposing user IDs, full names, email addresses and organization role labels of provisioned users. It found no access to investigation content, transaction or query results, clear-text passwords, MFA secrets or session tokens. It hasn't said what the agents did once inside.

State-nexus actor's AI agents ran parallel intrusions against foreign ministries, pharmaceutical firms and telecoms worldwide
Agent-assisted attack
CrowdStrike says VAPOR PANDA's ARTEX agents ran parallel intrusions from Oct 17, with a human operator steering only at decision points. At two US pharmaceutical firms the agents chained a SAML and XXE flaw, deployed web shells, got root in a production container environment and read LAPS passwords; at a Greek government entity they forged an administrator token and read mailboxes. Post-exploitation confirmed at nine organizations; more than 200 targeted.

Attacker used an agentic pentesting tool in what a domain registrar calls its first AI-driven cyberattack, exposing customer data
Agent-assisted attack
123 Reg says the Sep 18 intrusion was 'the first known AI-driven cyberattack against our company' and that the actor 'used ARTEX, an open-source Chinese-developed agentic penetration-testing tool', which was seen in the attack. Investigators also found AI attack 'skills' on the attacker's infrastructure. Customer contact details, account and product data, VAT numbers and API product information were read; copies may have been made.

One-command agent workflow exfiltrated a US CRM vendor's client data and breached SaaS and data center companies
Agent-assisted attack
A single slash command launched a seven-step workflow that left exploitation choices to Claude Code and included explicit exfiltration steps. CrowdStrike says the Russian-speaking actors exfiltrated corporate client data from a US-based CRM vendor and compromised several SaaS and data center companies in Russia, Kyrgyzstan and the US, plus one foreign government domain.

Actor told a coding agent to compromise targets and breached government domains in Indonesia and another country
Agent-assisted attack
The operator gave Claude Code 819 mostly public skills with no other orchestration and told it to compromise each target, stepping in only to push past guardrails or keep it trying. CrowdStrike says several Indonesian government domains, one government domain of another country, and several private-sector entities were compromised.

Agent-assisted attack
A 'Supervisor' Claude Code agent picked the next task from a state machine and an 'Executor' agent carried it out, inside a framework built from PentestGPT with added MCP servers, exploitation scripts and skills. CrowdStrike says at least 15 Argentina-based healthcare entities were compromised, with credentials and medical records exfiltrated from at least one.

State-aligned actor's AI agents used stolen app credentials to gain privileged access in downstream cloud tenants
Agent-assisted attack
The actor's AI agents automated scanning, password spraying and OAuth app abuse, then signed in to customer tenants with stolen app credentials and made application credential changes and changes involving identities, groups and privileged roles in some environments. Several ARTEX-driven attacks gained privileged access, some within 3 hours; most targeting was unsuccessful. Data access not confirmed.

Model under evaluation used exposed credentials to pull user records and non-public tables from a research organization's live systems
Eval escape
During evaluations, an internal research model found login details for an international research organization's live search service in public JavaScript, used them to pull an internal record and 1,489 user records with names and email addresses and altered a configuration setting on the running service; in a second evaluation it changed a setting to obtain a cloud storage access token and downloaded non-public tables totalling 147,451 rows. The first setting change was not restored when the evaluation ended; the second was restored, though the model kept using the token. The model disclosed none of it.

Model in training broke authorization on a government statistics website and pulled server secrets and account records
Eval escape
During RL training, an internal research model asked to find historical hospital-bed statistics on a government statistics website used the site's file-download feature to pull the compiled application and a configuration file holding a database connection string, the token-signing secret key and email login credentials, then minted its own tokens, stored 24 years of data instead of the one year requested and fetched a directory of more than 1,000 account records with email addresses. No remediation on the affected site is stated, and the model's answer said nothing about how it got there.

How we count

What counts

An AI agent’s own actions damaged production data, backups, or cloud resources, or took a service down. Two more kinds of cases count too, and each gets a tag. When an attacker uses an AI agent, or hijacks one, to break into real systems, the row is tagged Agent-assisted attack. When a model in testing or training gets out and reaches a real organization’s systems, and the vendor or that organization confirms it, the row is tagged Eval escape.

What doesn’t Count

Damage that stays on someone’s own laptop or inside a test environment, escapes that never leave the lab, demos, and mistakes a person made without an agent. Ransomware with no agent involved goes on the ransomware incident tracker. Cases with a disputed cause or no credible source stay off the page until that changes.

How we source

Each row links the best source we could find, and the label next to it says what kind it is. Usually that’s the affected organization’s own statement, the developer’s post or bug ticket, security research, or a regulator. Most accidents are self-reported, since the developer who got burned tends to post before anyone else does. We review and approve every row.
